Are you referring to the right service?
- This is a time-limited, goal-focused specialist rehabilitation OT service for patients living in the community who require intervention to maximise their independence and occupational performance.
- Clients (16 >) who do not meet the criteria for Community Rehabilitation Program (CRP) or Transitional Care Programme (TCP) or GEMITH
This service offers specialised assessment and treatment for:
- Cognitive, visual/ perception dysfunction
- Neurological disorders (CVA, SCI, ABI and neurodegenerative conditions)
- upper limb impairments; specialized hypertonicity management
- Acquired Brain injury / Traumatic Brain injury / Post-Concussion Syndrome
- Functional decline requiring specialty rehabilitation services
- Clients must be self-motivated to set goals and participate in an outpatient rehabilitation programme or have adequate support at home to enable this/follow–up with home programmes.
- Clients must be able and willing to attend the GCUH (5th floor block D) for therapy sessions – may need carer support if not independently mobile.
- This service is a service of last resort for patients eligible for care through DVA, Workcover, NDIS or relevant insurance. Your referral should be sent to these organisations in first instance and only referred to this service if clearly declined from the other organisation.
Alternate services may be more suitable for:
- Clients who meet the criteria for other outpatient and community rehabilitation services and would better meet their goals.
- Clients who have orthopaedic, peripheral nerve and traumatic upper limb injuries and who would be eligible for the outpatient hand and upper limb service.
- Clients who are unable to attend clinic based at GCUH or telehealth intervention and require home or community-based assessment and intervention.
Conditions
Out of Scope Services
The following conditions are out of scope for this service:
- Clients who meet the criteria for other outpatient and community services and would better meet their goals
- Clients who have orthopaedic, peripheral nerve and traumatic upper limb injuries and who would be eligible for the outpatient hand and upper limb service.
- Clients who are unable to attend clinic based at GCUH and require home or community-based assessment and intervention
- Driving assessment and intervention – this is currently not funded by the public system
Out of Catchment
Gold Coast Health is responsible for providing public health services to the people who reside within its boundaries. Special consideration is made for patients requiring tertiary services or for services that are not provided by their local hospital and health service district. If your patient lives outside the Gold Coast Health catchment area and you wish to refer them to one of our services, please indicate relevant medical or social information that will assist with the processing of your referral.
Notes
Please note that where appropriate and where available, the referral may be streamed to an associated public allied health and/or nursing service. Access to some specific services may include initial assessment and management by associated allied health and/or nursing, which may either facilitate or negate the need to see the public medical specialist.
- A change in patient circumstance (such as condition deteriorating or becoming pregnant) may affect the urgency categorisation and should be communicated as soon as possible.
- Please indicate in the referral if the patient is unable to access mandatory tests or investigations as they incur a cost or are unavailable locally.
- If your patient consents to telehealth, please indicate in the body of the referral.

- Any concession cards (e.g. Pension, Health Care, DVA, PBS Safety Net, ADF, etc.)
- Current medication list
- Reading glasses
- Any previously prescribed compression garments
