Category 1 (appointment within 30 calendar days) | Any medical comorbidities requiring subspecialty input or co-ordination during pregnancy including but not limited to:

Essential Referral Information
- Current pregnancy information (gravidity, parity and estimated date of birth) or identify if pre-conception review is required
- Past obstetric history (if known) and previous pregnancy complications
- Relevant medical, surgical, and psychosocial history
- Current medications
- Blood pressure and relevant clinical findings
- Routine antenatal bloods and other relevant pathology or imaging

- BMI
- Social history, where relevant
- Indigenous status, ethnicity and language spoken (identify if interpreter is required)
