Minimum Referral Criteria
Category 1 (appointment within 30 calendar days) | Children under 18 who are currently experiencing an inpatient admission who require daily support for the following treatments:

Essential Referral Information
- Patient requires daily treatment
- Diagnosis for CHITH treatment
- Admitting team and responsible consultant
- Medical history and history regarding inpatient admission
- Reason for Physiotherapy intervention (if Allied Health referral)
- Estimated date of transfer to CHITH
- Length of treatment required on CHITH
- Paediatric review and follow up plan documented
- Completion of Referral Form
