Are you referring to the right service?

  • H2HK-Q Gold Coast is a service that accepts referrals for children and families experiencing developmental, mental health, behavioural and emotional challenges, that impact on the child and family function both at home and in their school or social connections
  • H2HK-Q Gold Coast aims to provide services to those children and families with multiple challenges that are not currently having their needs meet by a service. The team at H2HK-Q Gold Coast will provide treatment and supports for your family’s needs and work together to connect you with the most appropriate healthcare services for ongoing support
  • All referrals to H2HK-Q Gold Coast will require parent/guardian consent
  • Children being referred to H2HK-Q Gold Coast must meet the following referral criteria:
    • Under 12 years old
    • The presentations being referred are with a duration greater than 2 months.
    • Mental health problems which:
      • are compromising the child/young person and their family’s quality of life.
      • impact on functioning across multiple domains (e.g., schooling, activities of daily living, peer/close relationships, activities, and hobbies)
  • Interventions at the primary care level have not been successful because:
    • presenting difficulties are not responsive to non-specialist support (e.g., school psychologist / guidance officer / social worker / community health nurse)
    • the child faces barriers to accessing primary care
  • For development concerns or assessment for ADHD, ASD or any developmental delay please refer to Child Development Services
  • For Feeding concerns please refer to Child Development Services
  • For Urinary incontinence or enuresis please refer to Paediatric Medicine
  • For sexual, physical or domestic violence please refer to Child Protection Unit
  • For medical related concerns or diagnosis please refer to Paediatric Medicine
  • For moderate to serve mental health issues, refer to CYMHS Access by calling 1300 MH Call
  • For Children over the age of 12 please call Head to Health phone service on 1800 595 212

Out of Scope Services

The following conditions are out of scope for this service:

  • Referrals for assessments with the sole purpose to access NDIS supports
  • Consumers currently linked with Child and Youth Mental Health Services
  • Children over the age of 12 years
  • Currently under the care of Child Development Services
  • Medication reviews
  • Diagnostic assessment of medical, developmental, or mental health conditions
  • Families or Children experiencing acute mental health crisis

Out of Catchment

Gold Coast Health is responsible for providing public health services to the people who reside within its boundaries. Special consideration is made for patients requiring tertiary services or for services that are not provided by their local hospital and health service district. If your patient lives outside the Gold Coast Health catchment area and you wish to refer them to one of our services, please indicate relevant medical or social information that will assist with the processing of your referral.

  • Medicare card
  • Any concession cards (e.g. Pension, Health Care, DVA, PBS Safety Net, ADF, etc.)
  • Current medication list
  • Reading glasses, hearing and mobility aids

If your patient does not meet the minimum referral criteria

  • Assessment and management information may be found on a range of conditions at HealthPathways
  • If the patient does not meet the criteria for referral but the referring practitioner believes the patient requires specialist review, a clinical override may be requested:
    • Please explain why (e.g. warning signs or symptoms, clinical modifiers, uncertain about diagnosis, etc.)
  • Please note that your referral may not be accepted or may be redirected to another service

Essential Referral Information

  • History of the current situation including:
    • Presenting problem
  • Relevant previous medical or mental health history
    • Current Medications (if relevant)
    • Relevant family violence or safety concerns or Child Safety involvement
  • Previous involvement in Parenting programs
    • Any current involvement of other services (e.g., Psychology, Occupational Therapy)
    • Next of Kin details including any court orders or custody considerations
  • Any restrictions that may impact access to the service (e.g., Physical impairment or lack of transport)
    • Name of current school and year including any relevant education history

Additional Referral Information

  • Reports completed by Guidance Officer or Wellbeing workforce (if available)
  • Reports completed by external allied health professionals
