Are you referring to the right service?

  • Advance care planning is the process of thinking about and communicating a person’s preferences for future health care.
  • Ask the clinical question: Would you be surprised if this patient died in the next 6-12 months? If yes – an advance care planning facilitation is required.
  • Royal Australian College of General Practitioners Advance Care Planning
    • “a process of reflection, discussion and communication that enables a person to plan for their future medical treatment and other care, for a time when they are not competent to make, or communicate, decisions for themselves”
  • The process of decision making is collaborative in consultation with clinicians, family members and important others.
  • Advance Care Planning Referral provides support to understand Queensland’s statutory and non-statutory documents available to Queensland citizens to complete for self-management of personal affairs. The conversation is person-centred for each individual.
  • The service is provided by a GCHHS Clinical Nurse Consultant - Advance Care Planning.

Essential Referral Information

  • Is the person over 18 years of age?
  • Does the person retain decision making capacity?
  • Has the person consented to have an advance care planning conversation?
  • Medical history
  • Current treatments
  • Active Acute Resuscitation Plan (ARP) in place
  • Are any Advance Care Planning documents complete?
  • Enduring Power of Attorney/ Power of Attorney
  • Advance Health Directive
  • Statement of Choices
  • Advance Care Planning Note – A written letter for care at end of life
  • Does the person identify as Aboriginal and/or Torres Strait Islander?
  • Does the person identify as Culturally and Linguistically Diverse (CALD) /Multicultural Community Gold Coast?
  • Does the person identify as GBLTIAQ+?
  • Does the person identify as having a disability

Additional Referral Information

  • What family support does the person have? They may also attend the consultation.
  • Has a ceiling of care pathway discussion occurred?

Out of Catchment

Gold Coast Health is responsible for providing public health services to the people who reside within its boundaries. Special consideration is made for patients requiring tertiary services or for services that are not provided by their local hospital and health service district. If your patient lives outside the Gold Coast Health catchment area and you wish to refer them to one of our services, please indicate relevant medical or social information that will assist with the processing of your referral.

Notes

Please note that where appropriate and where available, the referral may be streamed to an associated public allied health and/or nursing service. Access to some specific services may include initial assessment and management by associated allied health and/or nursing, which may either facilitate or negate the need to see the public medical specialist.

  • A change in patient circumstance (such as condition deteriorating or becoming pregnant) may affect the urgency categorisation and should be communicated as soon as possible.
  • Please indicate in the referral if the patient is unable to access mandatory tests or investigations as they incur a cost or are unavailable locally.
  • If your patient consents to telehealth, please indicate in the body of the referral.

  • Medicare card
  • Any concession cards (e.g. Pension, Health Care, DVA, PBS Safety Net, ADF, etc.)
  • Current medication list
  • Reading glasses, hearing and mobility aids
