The Refer Your Patient website will make it easier to refer patients between hospital and community-based services and ultimately:

• Enable better quality referrals
• Allow for equitable and accurate triaging
• Facilitate a patient’s improved readiness for care
• Promoting efficiency of referral processing by creating transparency of the essential referral information required

By creating a transparent view of referral criteria for all services offered across the organisation, all referrers will be able to access up-to-date information at the point of care, supporting stronger clinical decision making and care planning.

This website will enable General Practitioners, Optometrists, Private specialists and Internal referrers across a range of specialties to:

• Access current clinical referral criteria and service information
• Appropriately assess a patient’s clinical urgency when referring them for further treatment
• A comprehensive overview of service information including minimum clinical requirement for care and essential referral information required for the condition
