Existing referral templates for referral to Gold Coast Health are currently out of date and will be removed from the Primary Health Network website on 31 August 2022.

Smart Referrals is now the preferred method of making a referral into Gold Coast Health outpatient clinics.

If you have not had Smart Referrals installed, please contact our General Practice Liaison Unit who can assist you with installation and training.

The new platform provides all the criteria required for the specialty clinic and allows documents to be attached and sent with the referral.

You can also track the referral status and receive more communications via electronic transmission.

Nurse Practitioners and Midwives in general practice, who have a provider number, can also use Smart Referrals.
