IMPRINT aims to reduce potential hospital length of stay through early navigation and intervention of Gold Coast Health residents who

  • have recently been approved or are awaiting a high-level Home Care Package by the Aged Care Assessment Team (ACAT) and
  • would benefit from short term intervention in their own home by our multidisciplinary team including Geriatrician, GP with special interest, Occupational Therapist, Clinical Nurse Consultant, Social Worker and in the near future, neuropsychologist.

The Webinar will:

  • Outline the IMPRINT service and model of care.
  • Outline referral pathways and patient criteria.
  • Provide an educational introduction to other Gold Coast Health community services that may be of benefit to you and your patients.

Given the educational content around community services as well as case studies, this Webinar will be eligible for a self-recorded activity for your annual CPD.
