How to make a referral to VAD using Smart Referrals

To make a referral to the Gold Coast Health Voluntary Assisted Dying (VAD) Service using Smart Referrals:
1. In the “Condition and Specialty” field enter one of the following specialities and choose the option with no clinical condition attached:
- Cardiology
- Respiratory
- Neurology
- Oncology
- Haematology
- Palliative Care
2. Once the specialty is selected the Gold Coast Health Voluntary Assisted Dying Service will appear as a choice in the “Service/Location” field.
For GPs unable to use Smart Referrals please use your usual referral method and indicate that the referral is for VAD Service.
