Chronic Disease Complex Care Nurse Navigator

Nurse Navigators provide culturally safe care to Aboriginal and Torres Strait Islander patients with chronic disease, disease and/or complex medical concerns, in close partnership with multiple health specialists and health service stakeholders.


The Nurse Navigators assist these patients with complex care and support needs from the public health system and out to external non-government organisations to ensure patients receive the appropriate and timely care needed.
