Better Cardiac Care

This Program aims to improve access, support and knowledge to evidence based culturally safe cardiac care for Indigenous people.

The Mutli-discilinary Team (MDT) consists of Clinical Nurse Consultant, Aboriginal and Torres Strait Islander Health Worker and Pharmacist and review patients during the acute phase of illness:

  • to support and advocate for patients and families.
  • build trusted relationships in the hospital setting which facilitate engagement in community after discharge.
  • actively link the patient to GP, specialist and community support at the point of discharge.

From a prevention perspective, the team conduct cardiovascular disease risk factor assessments and provide education in the community by attending Aboriginal and Torres Strait Islander community events.
