A respectful and appropriate approach to advance care planning for Aboriginal and Torres Strait Islander people ensures their end-of-life journey is inclusive of their individual preferences, values and beliefs, and their cultural heritage.
GP Partnerships and Engagement Unit
This Program is for adults living in the Gold Coast catchment area, aged over 18 years who require specialist intervention for chronic heart failure.
The time-limited service offers a heart failure:
- medication titration clinic for clients who have a confirmed diagnosis of chronic heart failure
- exercise and education program with multidisciplinary input for adults over the age of 18 years who have a confirmed diagnosis of chronic heart failure (on echocardiogram within the last 12 months) and are managed by a public Gold Coast Cardiologist.
The service is located at both Robina Health Precinct and Helensvale Community Health Centre.
