This service is for patients 18 years and older with Chronic Kidney Disease (CKD) (eGFR<60ml/min) not requiring immediate Nephrologist review.

The service works with clients and GPs to:

  • slow the progression of kidney disease and prevent potential complications.
  • provide education, monitoring and support for clients with CKD.
  • host a support group every two months to promote self-management.

The team also works with other allied health services to manage chronic conditions that may affect or impact on CKD.
