- Detailed sleep history
- Assessment of relevant contributory factors such as psychosocial and environmental contributors
- Presenting complaint and reason for referral
- Summary of any contributing underlying conditions
- Parent has been provided with appropriate resources (see list below)
- Detail of interventions tried by health care professionals and timeframes for intervention
