Essential Referral Information
- Detailed history of nephrolithiasis episodes
- FBC, ELFT, urea, creatinine, eGFR, calcium, magnesium & phosphate results
- Ultrasound (kidney, ureters & bladder) or CT results
- Urine microscopy for evaluation of urinary sediment and urine albumin creatinine ratio
Additional Referral Information
- Family history of chronic kidney disease and/or calculi
- BP
- Record of previous urinalysis and urine biochemistry if available
- Record of any previous calculus biochemistry if available
- Stone analysis
