Essential Referral Information
- Details of any previous gastrointestinal or bariatric surgery
- Details of all treatments offered and efficacy
- Fasting glucose
- ELFT
If a specific test result cannot be obtained due to access, financial, religious, cultural or consent reasons a clinical override may be requested. This reason must be clearly articulated in the body of the referral.
Additional Referral Information
- Glucose at time of symptoms if available
- Concomitant insulin, c-peptide at time of symptomatic hypoglycaemia
- Morning cortisol/ACTH , TFT
