Minimum Referral Criteria

Does your patient meet the minimum referral criteria?

Category 1 (appointment within 30 calendar days)	Likely diagnosis of glaucoma and any of the following: IOP 30-35 mmHg severe disc damage severe field loss
Category 2 (appointment within 90 calendar days)	Likely diagnosis of glaucoma and any of the following: signs of early disc damage or field loss consistent with glaucoma suspicion of narrow iridocorneal angles with risk of angle closure glaucoma
Category 3 (appointment within 365 calendar days)	IOP ≥25mmHg OR IOP repeatedly 22-25mmHg WITH any of the following risk factors: central corneal thickness <510 microns history of blunt eye injury with hyphaema or vision loss pseudo exfoliation pigment dispersion uveitis First degree relative with glaucoma Optic disc margin haemorrhage Increase in measured IOP over time by 50% or more Medications at risk of increasing IOP (e.g. steroids)
If the patient does not meet the criteria for referral but the referring practitioner believes the patient requires specialist review, a clinical override may be requested: Please explain why (e.g. warning signs or symptoms, clinical modifiers, uncertain about diagnosis, etc.) Please note that your referral may not be accepted or may be redirected to another service

All conservative options that have been pursued unsuccessfully prior to referral
Body mass index (BMI)
Details of any associated medical conditions which may affect the condition or its treatment (e.g. diabetes), noting these must be stable and controlled prior to referral
Any special care requirements where relevant (e.g tracheostomy in place, oxygen required)
Current medications and dosages
Drug allergies
Alcohol, tobacco and other drugs use

Best Corrected Visual Acuity (BCVA) (vision with most recent distance spectacles)
Private ophthalmologist or optometrist report including VA, IOP, visual fields & disc assessment

Additional Referral Information

Optical coherence tomography (OCT)
Refraction, gonioscopy, pachymetry
Photograph – with patient’s consent, where secure image transfer, identification and storage is possible
