Minimum Referral Criteria

Does your patient meet the minimum referral criteria?
Category 1 (appointment within 30 calendar days)
  • Likely diagnosis of glaucoma and any of the following:
    • IOP 30-35 mmHg
    • severe disc damage
    • severe field loss
Category 2 (appointment within 90 calendar days)
  • Likely diagnosis of glaucoma and any of the following:
    • signs of early disc damage or field loss consistent with glaucoma
    • suspicion of narrow iridocorneal angles with risk of angle closure glaucoma
Category 3 (appointment within 365 calendar days)
  • IOP ≥25mmHg

    OR
  • IOP repeatedly 22-25mmHg WITH any of the following risk factors:
    • central corneal thickness <510 microns
    • history of blunt eye injury with hyphaema or vision loss
    • pseudo exfoliation
    • pigment dispersion
    • uveitis
    • First degree relative with glaucoma
    • Optic disc margin haemorrhage
    • Increase in measured IOP over time by 50% or more
    • Medications at risk of increasing IOP (e.g. steroids)
  • If the patient does not meet the criteria for referral but the referring practitioner believes the patient requires specialist review, a clinical override may be requested:
    • Please explain why (e.g. warning signs or symptoms, clinical modifiers, uncertain about diagnosis, etc.)
  • Please note that your referral may not be accepted or may be redirected to another service

Essential Referral Information

  • Best Corrected Visual Acuity (BCVA) (vision with most recent distance spectacles)
  • Private ophthalmologist or optometrist report including VA, IOP, visual fields & disc assessment

Additional Referral Information

  • Optical coherence tomography (OCT)
  • Refraction, gonioscopy, pachymetry
  • Photograph – with patient’s consent, where secure image transfer, identification and storage is possible
