Fractional Exhaled Nitric Oxide (FeNO)Adult
Useful Management Information
The FeNO test can detect eosinophilic airway inflammation, determine the likelihood of corticosteroid responsiveness, monitor airway inflammation to determine the need for corticosteroids and identify non-adherence to corticosteroid therapy.
Indications:
- Assess the etiology of respiratory symptoms.
- Help identify the eosinophilic asthma phenotype.
- Assess the potential response or failure-to-respond to anti-inflammatory agents, notably inhaled corticosteroids.
- Establish a baseline FeNO during clinical stability for subsequent monitoring of chronic persistent asthma.
- Guide changes in doses of anti-inflammatory medications step-down dosing, step-up dosing or discontinuation of anti-inflammatory medications.
- Evaluate adherence to anti-inflammatory medications.
- Assess whether airway inflammation is contributing to poor asthma control, particularly in the presence of other contributors (e.g. rhinosinusitis, anxiety, gastro-esophageal reflux, obesity and continued allergen exposure).
Clinical advice:
- Cigarette smoking suppresses FeNO which limits its use in smokers.

- Infectious status
If a specific test result is unable to be obtained due to access, financial, religious, cultural or consent reasons a Clinical Override may be requested. This reason must be clearly articulated in the body of the referral.
